Dr SUSAN BARBARA RUTKOWSKI, Longueville
For service to medicine in the area of spinal cord rehabilitation as a clinician, mentor and researcher, and through contributions to advocacy groups and charitable organisations.
Director, Spinal Cord Injuries Outpatient Services and Research, Royal North Shore Hospital, since 2003; Director, Spinal Cord Injuries Service, 1992-2002; has developed fertility assistance service for people with spinal cord injuries since 1987.
Clinical senior lecturer, Department of Medicine, University of Sydney, for many years.
Current Associate Director, Statewide Spinal Cord Injuries Service; prior to the establishment of this service, Dr Rutkowski made regular visits to rural centres to meet with medical staff and patients to assist with rehabilitation.
Current honorary medical officer at the Royal Ryde Rehabilitation Centre, The Children’s Hospital at Westmead, Ferguson Lodge at Lidcombe, and the Lottie Stewart Hospital at Dundas.

MATTHEW JOHN MITCHAM, Balmain
For service to sport as a Gold Medallist at the Beijing 2008 Olympic Games.
Mr Mitcham won a Gold Medal at the Beijing 2008 Olympic Games in diving in the Men’s
10 metres Platform. He represented Australia in an exemplary manner and brought great credit to himself, the Australian team and the nation.

GRANT KARLIS MIZENS, East Ryde
For service to sport as a Gold Medallist at the Beijing 2008 Paralympic Games.
Mr Mizens won a Gold Medal at the Beijing 2008 Paralympic Games in the Men’s Wheelchair Basketball. He represented Australia in an exemplary manner and brought great credit to himself, the Australian team and the nation.

AMBULANCE SERVICE MEDAL (ASM)
JENNIFER VAN CLEEF, Epping

Mrs Van Cleef commenced her ambulance career in 1992 and was stationed at Lane Cove. She worked within the Sydney metropolitan area before transferring to the Sydney Operations Centre where she was promoted to Station Manager. In 1996 Mrs Van Cleef was appointed District Superintendent of the Centre. Mrs Van Cleef’s talent in project work was recognised in 2004 when she was promoted to Superintendent and made responsible for the project management of the development, implementation and ongoing management of the Patient Clinical Allocation Matrix 2004-2007 and Ambulance Status Board. The project represented a significant change to work practice and required extensive consultation, negotiation and coordination of a large, diverse group of stakeholders and resources.

PUBLIC SERVICE MEDAL (PSM)
UNIS YIM-WAH GOH, Pymble

For outstanding public service, particularly in the provision of community housing in New South Wales.
Mrs Goh’s career in the NSW Public Service spans over 34 years in a range of positions in the Department of Community Services and, more recently, the Department of Housing. Throughout her career, Mrs Goh has demonstrated strong leadership and management.

RODNEY DALE MORRISON, Breakfast Point
For outstanding public service, particularly in the development of enhanced safety measures within the mining industry in NSW.
Mr Morrison has had a highly distinguished career in the NSW Public Service spanning over 44 years. Commencing his career with the NSW Film Council, he has served in various government agencies including the Department of Community Services. His high level of commitment and ability to deliver changes and improvements to government administration and efficient service are typified by the changes he introduced and managed in industrial relations, including the innovative program of Flexible Work Hours, development of the Senior Executive Service and the establishment of a human resources strategy for the Department of Community Services. In his current role as Director, Mine and Forest Safety Division, Department of Primary Industries.
